5Essentials Survey
From now until March 28, 2025 parents across Illinois will participate in the annual statewide Illinois 5Essentials Survey. This survey is designed to generate a detailed picture of the inner workings of each school. This opportunity will allow you to share your thoughts on the important elements of school effectiveness in a survey specific to your child’s school of attendance.
The Illinois 5Essentials Survey is administered online by UChicago Impact at the University of Chicago on behalf of the Illinois State Board of Education. The survey gathers data related to five indicators that can predict important student outcomes, including improved attendance and larger test score gains.
These five indicators that affect and predict school success are:
• Effective Leaders
• Collaborative Teachers
• Involved Families
• Supportive Environments
• Ambitious Instruction
Each of our schools needs at least 20 percent of its parents to complete this survey so that a parent supplement report can be generated for the school. Your identity and survey responses will be kept completely confidential and will never be connected to you or your child. If you have children in multiple schools and you want to provide input for each school, you will need to complete a separate survey for each, as the survey is an individual school survey and not a district-wide survey.
To take the survey please visit http://survey.5-essentials.org/illinois/ and select the appropriate survey to begin.
